The ReMend Process.

Once you’ve made the decision to communicate with a ReMend Mentor, you can complete a referral here or reach out to your social worker at your dialysis unit, your social worker will send a referral to ReMend. 

A ReMend Intake Specialist will contact you and ask a few questions about your kidney journey including why you reached out to ReMend. This will help in finding a mentor to meet your needs. Once a Mentor has been matched, they will reach out to you directly. You have several options when communicating with your mentor:  by phone or via an online conference service like Zoom or Microsoft Teams, or a meet up at a local coffee shop, etc. We want to make it easy for you!

Currently, due to COVID-19 social distancing, all Mentors contact patients via telephone, or teleconferencing services.
